What is a Fishbone Diagram?

Also known as an Ishikawa or cause-and-effect diagram, it helps identify potential causes of a problem by organizing them into categories. Common categories include the 6Ms (Machine, Method, Material, Measurement, Man, Environment) for manufacturing or 4Ps (Policies, Procedures, People, Plant) for service industries.
